PEDIATRIC ASTHMA DAY CARE PROJECT

QCPC has been designated by City Councilman Leroy Comrie, as one of two Queens community-based organizations to implement health tracking and training activities to address asthma in Group Day Care, Pre-Kindergarten and Head Start facilities in South Jamaica, Jamaica and on the Rockaway peninsula.

The goal of the Pediatric Asthma Day Care Project is to decrease hospitalizations, emergency department visits and school/work absences related to asthma. QCPC will provide technical support to 10 sites where linkage agreements have been established, representing at least 500 enrolled children, to enhance their health record maintenance system, ability to identify children with asthma, and track each child’s progress in managing asthma through the use of a health tracking database. QCPC will also work with each facility to develop site-specific asthma plans.

Asthma training will be provided to at least 100 group day care and Head Start staff including administrators and teachers. Asthma training will also be provided for parents of the children enrolled at each site.

Group meetings will be convened among all sites to discuss progress and barriers and share best practices and lessons learned from program implementation.
